Note: The Vaccine Information Sheet (VIS) is federally mandated to be given prior to the administration of a vaccine. The fact sheets are provided as supplementary materials and can be used in addition to the VIS but not instead of the VIS. A VIS still must be given when a vaccine is administered.

For parents of infants and young children (birth through 6 years old)

There are 2 versions of each fact sheet. The "Disease Basics" include an overview of each vaccine-preventable disease and vaccine information. "Disease Basics" are available in Spanish as well as English. The "In-Depth Information" sheets use a narrative style and some include a true story about a family affected by a vaccine-preventable disease.
