Register for Free AHRQ Webinar on Shared Decision Making for Chronic Conditions and Long Term Care Planning

A webinar from the Agency for Healthcare Research and Quality (AHRQ) on July 26, 2016 will examine how shared decision making can be used to manage chronic conditions and facilitate the long term care planning needs of aging Veterans and adults with chronic conditions. Experts will address: (1) how Dartmouth’s School of Medicine is using shared decision making to improve care management of individuals with chronic conditions, and (2) how the Veteran’s Administration is implementing shared decision making to engage Veterans in their long term care planning decisions.
